Employee’s statement of sickness to claim Statutory Sick Pay

You can use this form to ask your employer for Statutory Sick Pay (SSP). You must have been off work due to sickness for more than 3 days in a row to be eligible. When you’ve filled in the form give it to your employer, it will help them decide if you can get SSP. Keep a copy for your own information.

Statement of Fitness for Work (Sick Lines)

If you have been discharged from hospital, it is the hospitals duty to provide you with your Med 3 form (sick line) if you are unable to return to work. The GP will no longer issue a sick line for those patients. Please click here for further details.

If you are chronically ill and have not been treated and discharged from hospital and your sickness continues for more than 7 days you will require a doctors statement of fitness for work, also known as a sick line. Sick lines can only be requested on the day they expire. They can not be post dated. 

Please call us from 10.30am on the date of expiry of the sick line to request a new one.
